D-SUB "Out of Range"

So recently my monitor has been messing up. It started with changing the resolution on its on. Then it started going to a grey screen that said D-SUB Out of range 75hz/60hz. I've tried unplugging the monitor cables, turning the computer on and back off and many more things. I even went into safe mode but when I booted it into safe mode my resolution was wrong and it wouldn't allow me to change it. Please help, I have no idea what to do now. FYI: My monitor is a 1920x1080 LG monitor, not sure which model. Not even a year old.
