How to skip blanks to generate a table (Excel)

I have a set of data which I am placing into a table (see attached doc, Samples tab). If there is a sample present, I need to represent it by the sample number and test type (see 'Data for Table' tab). I have generated this list, but also have a lot of blanks. With the data that exists, I need to automatically combine it into single column with no blanks. This data then populates a table in the third tab.

This file is for someone who knows nothing about excel. He needs to enter the number of samples and have a table automatically generated. He cannot sort or move selected text boxes.

Can anyone help? Thanks!!
